This is the perfect role for an existing Care Co-ordinator, Care Supervisor or a Care Assistant looking to progress their career!

The Co-ordinator holds one of the most important positions in First City as all your actions directly affect the happiness and wellbeing of both our customers and our staff.

The purpose of the Care Co-ordinator role is to make sure our customers receive excellent quality care and support to help them remain safe and comfortable in their own homes.

You will be joining a team of friendly and kind-hearted professionals who are proud of the work they do, We provide training and ongoing support to ensure you understand our carers and our customers. Our Care Co-ordinators are part of the care team and would be expected to help with care calls where necessary and partake in out of hours on call duites.

Location: Swindon

Hours: Monday-Friday 08:30am -17:00pm. Some flexibility in hours may be required to suit the needs of the business

Salary: £23400 per annum, additional earnings available when partaking in out of hours duties

Key responsibilities of a Care Coordinator:

  • Allocation of care workers to customer visits using effective rostering, ensuring continuity of care and considering locations, skills required.
  • Communicating regularly with care workers, providing guidance and resolving queries in a timely manner
  • Maintaining telephone contact with customers and staff members
  • Ensure telephone calls are answered in a timely, polite and professional manner
  • Ensure all paperwork is completed and entered onto relevant systems
  • Support with the delivery of the on-call rota, dependent on the needs of the business. This will include being the designated contact for the out of hours teams during evening/weekends and supporting all aspects of care delivery.
  • Support the field team covering customer visits to support the business needs when required.
  • Be the 1st line of support for carers and clients with care related enquiries
  • Ensure compliance is met within the office and the care calls
  • Assist and support the manager
  • Maintain integrity when dealing with confidential matters
  • Provide supervisory support and instruction to care staff
  • Adhere to company policies
  • Approach clients with understanding and empathy, remembering that each client is to be treated as an individual
  • Work as part of a team that revolves around person-centred care and support

The successful candidate will:

    The Role

    We have an exciting opportunity to join an existing team to provide waking night support for an individual within her own home, ensuring her safety and well-being throughout the night.

    You will be working in a specialized 3 – 1 package to support a young lady with her changing needs due to her Autism, Epilepsy and behaviours that challenge.

    Full specialised training is provided for any appointed applicant whether you have experience in the industry, supporting someone with Autism or no experience.

    We are looking for a candidate who is naturally passionate about caring and able to promote and encourage independence. You will be required to work as part of a team and follow the service user's outcome-based support plan under the guidance and direction of the management staff. 

    Positions available: Full Time Nights and BANK

    Hours:  WAKE NIGHTS – 22:00pm – 7:00am

    Rate: £12.68ph

    Our customer has a great sense of humour and loves to build rapport with new people in her team, so she’s looking for someone who she can have a laugh with. While she is very social and enjoys activities such as going to farms, walks, clubbing, and attending friendship circles, your role during waking nights will focus on ensuring her safety, well-being, and comfort throughout the night.

    She experiences seizures due to Epilepsy and while they may not be frequent, they do require rescue medication and, at times, hospital support. Being able to recognize and respond to her unique communication signs is essential in providing effective support and anticipating a seizure or any triggers that could lead to behaviours that challenge.

    Your Role at Waking Night colleague:

    • Regular physical monitoring every 10 minutes throughout the night
    • Constant observation through a monitor
    • Keeping living environment tidy and maintaining a calm atmosphere.
    • Alert sleeping colleagues to ensure safe and effective support if customer wakes
    • Providing assistance when required for personal care, though the customer is largely independent
    • Supporting with the evening and morning routines

    Although this is an established team, our customer’s needs are evolving, so we require individuals who can adapt to any changes. Due to the specialist nature of this role, the successful candidate must also be prepared to work within other areas of the business in and around Swindon if needed. 

    Essential –

    • Must have flexibility and willingness to work varying shifts, including weekends
    • Be a team player

    Desirable-

    • A full valid UK driving licence and be happy to drive our customers vehicle
    • Experience working with individuals with Autism, Epilepsy and challenging behaviours
