If you've made a phone call to First City, it's likely you've spoken to one of our Care Coordinators! With the on-call to cover evenings and weekends, there is always a Care Coordinator at the end of the phone to assist both customers and colleagues when they need it.
Our Care Coordinators ensure rostering of customer visits meets the needs of each individual we support, ensuring enough time between medication visits, bearing in mind a customer's time preference and logically mapping out locations based on geographical routes. They are adaptable to urgent and unforseen circumstances, which may include customers requiring medical attention and their Care Worker needing to stay longer, covering last minute sickness or finding a way to roster for a new hospital discharge package.
Knowledge of the healthcare industry is very important within this role to be able to offer support and advice to colleagues calling in with queries; many of our Care Coordinators began as Care Workers, and some still provide hands on support alongside their office role.
Not only do out Care Coordinators support our colleagues and customers, they also liaise with families/friends/next of kin (where appropriate) about their loved one's care, which can include helping reslove any complaints and queries they may have, passing on positive feedback and emotional support. They are also often contacting other professionals across the health and care industry, for example contacting district nurses regarding a bed sore, speaking with the emergency services should a customer require them, and liaising with the hospital team to organise a customer's safe dischearge. It is therefore imperative they have excellent customer service skills and are able to prioritise their workload.
Alongside our Care Workers, Care Coordinators hold a lot of responsibility upon their shoulders as they ensure safeguarding is practiced for every person while also ensuring their wellbeing, both mentally and physically. It is important for Care Coordinators to be vigilant during every conversation and when reviewing Care Workers note keeping, in order to assess if there are any concerns which will require reporting and escalating to appropriate avenues, which can include Senior Managerment, the Council and CQC.
Compliance is key across First City; our Care Coordinators ensure our Care Workers are safe to support customers by monitoring training requirements and booking colleagues onto training courses, while working closely with our in-house Training and HR Colleagues.
It takes a committed person to be a Care Coordinator; someone who is kind, caring, compassionate, honest and truly wants to make a difference to the lives of others. Though it can be a tough role, sometimes with long hours, they see the fulfilment and career satisfaction, knowing they have played a part in making a difference to so many people in our local community.
