One Hour of Care - Brokerage

A Brokerage colleague acts as the bridge between a person and their support package. Offering independent advice and guidance so each person can make the right choice for themselves of their loved one. Ensuring confidentiality and privacy is maintained at every stage, all of our Brokerage colleagues are friendly, welcoming and responsive, with a wealth of knowledge about the healthcare market.

Our Brokerage team are in constant communication with partners across the industry, including the local hospital. With these close partnerships our team are able to locate availability with a care provider in order to allocate support packages to individuals in a timely manner, while ensuring care and support received by customers is from a reputable, regulated service. The hospital contacts our team of Brokerage colleagues when a person is fit for discharge, sometimes this can involve forward planning, however on some occasions it can mean fast thinking to locate a suitable support package to start the same day.

As well as communicating with external partners, it is key for the Brokerage team to work closely with internal colleagues across First City to ensure they are familiar with available capacity in our various departments and future capacity forecast through recruitment efforts. Crucially they must keep in touch with our Intermediate and Reablement Service who are able to provide immediate support to those in the community and hospital discharges until a full-tome support package can be sourced and implemented.

It takes a compassionate individual to work within the Brokerage role; someone who is kind, caring, honest and truly wants to make a difference to the lives of others by ensuring support is implemented smoothly for every individual.

Without the Brokerage team the hospital could struggle to discharge people who no longer require hospital admission and some individuals may be unable to source an appropriate support package
