It is crucial for all individuals who work within and are associated with health and social care providers to understand the importance of maintaining skin integrity, implement strategies to promote and preserve it and know how to respond to any issues arising.
Skin integrity involves the skin being whole, undamaged and intact. When skin integrity is compromised, it can lead to skin breakdown, resulting in wounds, ulcers, or pressure sores. These conditions can cause discomfort, pain, and may lead to serious infections, including sepsis, if not properly managed.
Several factors can affect skin integrity, including age, nutrition, hydration and underlying health conditions. For instance, older adults have thinner skin that is more prone to damage. Similarly, malnutrition and dehydration can make the skin dry and brittle, making it more susceptible to injury.
Health and social care providers play a crucial role in maintaining skin integrity. Clear processes and guidance should be in place and accessible to all involved in a customer's support. Care workers should conduct regular skin assessments during care visits to identify any changes or damage early. This includes looking for and reporting signs of redness, swelling, or skin breakdown when supporting with personal care. Talking to customers and asking if they have any pain or marks on their skin during a visit would also be beneficial if personal care is not required.
